Key takeaway
How private health insurance works in Ireland, who the main providers are, lifetime community rating, and whether it's worth getting.
Why people get private health insurance in Ireland
Ireland's public health system is free for certain services but comes with long waiting lists for consultants and elective procedures. Private health insurance gives you faster access to private and semi-private hospital rooms, shorter waits for consultants, and more choice over where and when you're treated.
Lifetime Community Rating
This is the most important thing to understand when you arrive in Ireland. Under Lifetime Community Rating, the price of private health insurance increases by 2% for each year you're over 34 when you first take out a policy. If you're 35, you pay an extra 2% loading. At 45, you pay 22% more than someone who took out insurance at 34 or younger.
The key rule: if you arrive in Ireland and are aged 35 or over, you have a 9-week window to take out insurance without paying the loading. After that window, the loading applies. This is a significant financial consideration for many new arrivals.
The main providers
VHI Healthcare — the largest provider, with the widest network of hospitals. Generally good for families.
Laya Healthcare — known for competitive pricing and good customer service. Strong digital tools.
Irish Life Health (formerly Aviva) — smaller network but competitive on some plans.
All three are regulated by the Health Insurance Authority (HIA) and must offer community rating (meaning they can't charge you more based on your health status or age within their plans).
How to choose a plan
The key factors to compare are: excess per night (what you pay per night in hospital), what consultants and hospitals are covered, whether it covers GP visits, and the annual premium. The HIA's free comparison tool at hia.ie lets you compare all plans side by side.
Is it worth it?
For families with young children or people with existing health issues, private insurance tends to be valuable. For young, healthy individuals, it's more of a judgment call — but given the loading system, taking out a basic plan early and upgrading later is usually the financially sensible approach.
